Patient Rights and Responsibilities

UHS is dedicated to your health. Our staff values your active participation in your health care and encourages you to exercise your rights and responsibilities.

 

Your Rights are to:

  • Be treated with consideration, dignity and respect in a private, safe, supportive and smoke-free environment.
  • Receive caring, respectful healthcare and services regardless of race, sex, color, religion, creed, national origin or ancestry, age, marital status, sexual orientation, disability, special disabled veteran and Vietnam-era veteran status, source of payment, political beliefs, and height or weight. When receiving UHS services, individuals can expect competent, humane treatment that respects their opinions and beliefs and promotes a positive self-image.
  • Have privacy.
  • Know the professional title of the person(s) providing care for you at UHS.
  • Receive complete and current information regarding the diagnosis, evaluation, treatment, and prognosis of your physical or psychological condition from your clinician or health professional, in terms that you can understand. When it is medically inadvisable to give such information to you, the information should be provided to a person designated by you or to a legally authorized person.
  • Participate in decisions involving your health care in a collaborative working relationship with your clinician or health professional, except when such participation is contraindicated for medical reasons.
  • Be informed of services available at UHS, including UHS after-hours and emergency care services.
  • Refuse treatment to the extent premitted by law, and to be informed of the potential consequences of the refusal.
  • Review the credentials of UHS clinical staff.
  • Change your assigned clinician or health professional if other appropriate professionals are reasonably available.
  • Reasonable continuity of care, within the limitations of UHS available appointment times.
  • Receive complete information and explanation concerning the need for, and alternatives to, a referral or transfer to another health care facility.
  • Access the UHS payment policies, fees, and to receive an explanation of any bill from UHS.
  • Access and inspect your health record and obtain copies. You have the right to request an amendment to the record.
  • Be informed regarding (1) appropriate procedures to offer suggestions to the UHS, and (2) policies regarding grievance procedures and external appeals, as required by state and federal laws and regulations.
  • Request Advance Directive materials (provided by the Michigan State Long Term Care Ombudsman Program).
  • Receive marketing and advertising materials that appropriately represent the competency or capabilities of UHS.

Your Responsibilites are to:

  • Keep your appointments at UHS, and if you cannot keep an appointment, notify UHS as early as possible.
  • Be considerate of the rights of other patients and respectful of UHS personnel and property.
  • Communicate directly and honestly about your medical history and healthcare.
  • Understand your health problems and to seek clarification of information you do not fully understand.
  • Tell your clinician or health professional if you are unable or unwilling to follow the prescribed treatment plan.
  • Be familiar with the names and uses of the medications you are taking.
  • Tell your clinician or health professional about any changes in your health.
  • Provide information regarding your health insurance coverage and for promptly paying bills for services received at UHS.
