| You are expected to complete and return the following
forms at least 30 days before classes start (use
Adobe
Acrobat Reader for these PDF files):
- UHS Immunization Form (for all new students; should be completed by a health
care provider)
- UHS Insurance Form - note: all students should complete EXCEPT:
- International students
- Students without health insurance
- Graduate students who will receive health insurance (currently GradCare) as a benefit of employment or fellowship at UM (that is,
students funded by their program)
- Authorization for Consent
to Treatment of a Minor Form (only for students under the
age of 18, and your parent or guardian must complete this form)
- Tuberculosis screening is required for certain incoming international students. See
Tuberculosis Screening Requirement.
To help you stay healthy at school, all your immunizations should be current. For frequently asked questions about

Students should have health insurance (and health insurance is required for international students and their dependents).
If you need health insurance, please see Insurance for Students
for options. Please become familiar with how your health insurance works, and carry your insurance card with you at all times.
A physical exam is not required by the University of Michigan, however athletic teams and clubs may have different
requirements. Check with the team or club to be sure.
If you have a complex health history, ask your clinician to provide
a letter summarizing your health history and treatments. Schedule
an introductory visit with a UHS physician and bring this letter
to your first appointment (do not send it in advance). Carry a list of medication allergies
with you.
You may wish to consider purchasing the Tuition Refund Plan, which significantly
reduces financial loss if students are unable to complete a term due to illness (physical or psychological).
Especially for new international students
In addition to the information above, the following information is relevant to international students.
Requirement: Certain entering international students are required to be screened
for tuberculosis. See Tuberculosis
Screening Requirement
International students and their dependents are required to have
health insurance. Health insurance is offered through the International
Center.
UHS offers language interpretation. Please inform our staff if you would like this service.
